Blame-Storming

Pun on brainstorming that describes the tendency of some organizations, when problems arise, to focus on fixing blame first, rather than solving the problems. The approach is often seriously counterproductive as members of the organization limit their communication to avoid self-incrimination and, because it may target and distract the individuals most necessary to solve the problem, since their closeness to and potential responsibility for it reflects their better knowledge of its nature, causes and cures.
